]]>Hernias of the anterior abdominal-wall after previous surgery, known as “Incisional Hernias” develop due to a loss of integrity and strength of the anterior abdominal-wall, involving the anterior rectus sheath and its lateral extensions.
How can it help?
This is due to the incision used in previous surgery causing a weakness in the sheath, often with a protruberant mass seen at the point of weakness and with the risk of entrapment of this mass at the site of the hernia.
An additional benefit is the improved abdominal contour due to the wide mobilisation and tightening of the anterior abdominal sheath. Further, this is a mesh free technique of repair and has a much lower risk of recurrence or complications due to the use of foreign material such as a mesh.
Procedure
The usual treatment involves exposing the area of weakness, dissecting out the hernia, reducing its contents and then repairing the area of weakness, often with the use of a synthetic mesh.

You are likely to experience some discomfort and swelling after a tummy tuck. The pain can be controlled with pain relievers. Initially, drains will be placed in the abdominal cavity to remove excess body fluid and reduce swelling. Wounds are closed with dissolving stitches under the skin and surface stitches which are removed at about a week after surgery.

Immediately following surgery, you will have surgical dressings on the wound. Once drains are removed, you will need to wear compression garments for three months.
The critical healing period is the first three weeks. Over this period, you will be asked to restrict your activities significantly. Dr. Arianayagam recommends time off work during this critical recovery period, and you will need help from your family.
With a full abdominoplasty, the cut and final scar will go from hip to hip, most commonly just over the pubic area. There is also a scar around the belly button. These scars will be visible, although less so after several months and are covered by a bikini.

As with all surgical procedures, there will always be risks such as post-operative bleeding, allergic reactions, infection, deep vein thrombosis, and haematoma.

Of the congenital types, the Penoscrotal web is particularly difficult to cope with as it makes the penis look extremely short and interferes with penetration during sexual intercourse.
Since 1980, when I came across a description by Hirshowitz et al in The “Hand” describing a combined Z Plasty / Y-V Advancement 5 flap technique, I have used the technique in webs in various parts of the body and have had excellent results.
