The visible wrinkles that form on the face are the result of habitual and usually uncontrollable contraction of some tiny muscles in the face, creating lines and creases in the skin.

How can it help?

Anti-Wrinkle injections use neuromuscular blocking toxins to stop those muscles from contracting. The toxin attaches itself to the neuromuscular endplate (the connecting point of the nerve fibre to the muscle fibre). Acetylcholine, the neurotransmitter that normally causes muscle contractions to occur, is prevented from reaching the nerve fibre. The muscle is effectively paralysed. The treatment is particularly effective in treating deep, central frown lines and nasolabial (cheek) folds and “crow’s feet” (smile lines) at the side of the eyes. Dr. Arianayagam uses the brands Botox or Dysport for anti-wrinkle injections.

During the initial consultation Dr Arianayagam will carefully and thoroughly examine your face, mapping out the area to be treated by examining your ability to move certain facial muscles. He will also ask you a variety of questions about your current health situation, medications, allergies and any past medical treatments. The treatment is done in the Scarba Street Clinic. Based on the plan agreed at your initial consultation Dr. Arianayagam will mark strategic points on your face. A large number of small injections are made to target the muscles causing your wrinkles. The injection will only take about 10 minutes.

Recovery

Anti Wrinkle injections require almost no downtime. You should be able to return to work the same day as your treatment. There may be some redness and swelling in the injection area, but this usually subsides very quickly. It will take 3-7 days for all swelling to subside and the end result to become apparent.

Frequently Asked Questions

Both Botox and Dysport are highly toxic drugs but are used in very small safe doses in anti-wrinkle injections. This treatment has been proven safe and effective over years, and the vast majority of patients experience no complications.

The effect of the treatment will last 6-8 months. As muscle action gradually returns, the lines and wrinkles will begin to reappear and need to be treated again. Over time the wrinkles may appear less severe as the muscles are trained to relax.

Common side effects include temporary bruising from the injection area, headache and mild flu like symptoms, all of which should resolve within 72 hours.

An occasional problem is drooping of the upper eyelid (ptosis) due to the toxin running from the forehead into the upper eyelid.  While this is a nuisance, full recovery is usually seen in 2 to 3 weeks.

Birthmarks arising from blood vessels (vascular malformations and haemangiomas) are the most common birth defects seen in children. When the problem is present at birth, it is known as a vascular malformation and it grows very slowly, in keeping with the increase in size of the body.

How can it help?

While best treated early to avoid the complications that could arise with increase in size, there is no urgency in this situation. It is a very different story with an haemangioma, which is first noted as a little red dot, about 3 weeks after birth and increases significantly in size and colour over the next 2 months. If you are concerned about a birthmark on yourself or your child please arrange an initial consultation with Dr Arianayagam who will make an assessment and provide you with recommendations.

Procedure

The traditional approach is one of “waiting and watching” and in a large percentage of cases, the growth stops by 6 months and by 2 years most of the haemangioma has disappeared. However, the problem is the unpredictability of the growth in the first 6 months, with some haemangiomas increasing in size by 1000 times making it too difficult to treat and causing severe problems to the patient and parents. The better approach would be for close monitoring of the problem, and if size increase is noted, for early intervention and removal of the problem with surgery or with laser or IPL treatment. This short-circuiting of a potential major problem is highly recommended to avoid getting into difficult and untreatable situations.

How can it help?

For a satisfactory outcome, excision needs to be followed by a strict and planned period of earlobe compression up to 6 months.

This is a difficult site to keep compressed and works best with a pair of custom made earrings. For about 10 years we have had excellent, attractive silver earrings made by Mac of Mac Art Jewellery in Bellingen. The earrings use two connected discs with a tightening mechanism which allows adequate pressure to be maintained.

Procedure

To achieve the best results, each custom made pair of earrings are made before scheduling the surgery as the compression must be started a week after surgery.

As in all keloid scar management, compression therapy is usually very effective. A bonus is that the patient is the proud owner of a new attractive pair of silver earrings!

Melasma , an acquired disorder of pigmentation due to hyperactivity of melanocytes ( the cells that produce melanin) caused by ultraviolet light stimulation and Post Inflammatory Hyperpigmentation ( caused by an over reaction to skin trauma ) are common problems of excessive pigmentation of the facial skin.

How can it help?

Though the problem is common and multiple treatments available, the consensus is that there is a need for a combination of treatment methods, with an initial intense treatment  period followed up by long-term maintenance treatment.

Treatments

PHOTOPROTECTION  is the cornerstone of all treatments, to protect the skin from the effects of ultraviolet light ( UVB and UVA) and visible light, all of which stimulate melanocytes and in some instances overstimulate them. Broad spectrum skin protection against UVA ( 315 – 400nm ) and UVB (290 – 315 nm ) is critical. In addition, adequate clothing and hats add to the protection.

TOPICAL THERAPIES

Hydroquinone (HQ)  is a widely used topical depigmenting agent ( acting by reducing the amount of melanin produced ) and  works best when combined with a Retinoid  (which inhibits melanin transfer into epidermal cells and increases pigment removal by increasing the turn over  of pigment containing epidermal cells ) and a Corticosteroid.

The combination treatment is initially for daily use for up to 3 months, followed by a long-term maintenance therapy.

The Nd: YAG LASER

This laser has a longer wavelength ( 1064nm) and penetrates deeper while sparing the surface epidermis, making it safe to treat darker skin types and  deep dermal pigmentation. The treatment is quick and well tolerated and with no”downtime”. It is repeated at for weekly intervals for at least 10 sessions of treatment.

“Leucoplakia “ (Leuco = white / plakia = patch) refers to the condition often seen on the lips and the oral mucosa (vermillion). This is a pre-cancerous condition and is the result of exposure to irritants (the sun on the lips and hot foods on the oral mucosa) causing a thickening and whitening of the lips or oral mucosa.

How can it help?

In effect, this is the formation of a protective callus on a damaged area. While this is protective, in time the area starts breaking down and ends up as a Squamous Cell Carcinoma.

Removal of the irritant will help but often treatment is necessary. In earlier days, surgical removal of the vermillion was the only option but over the last 25 years, Carbon Dioxide ablation of the lip mucosa (vermillionectomy) gives very good results with no scarring. A bonus is the improvement in the tone of the lip and colour of the vermillion.
